New: Attestation for Student Return to School Following an Illness

Dear TLDSB Families with students learning in school buildings

 As part of our effort to keep our school communities safe and prevent the spread of COVID-19, a new “Attestation for Student Return to School Following an Illness” form has been developed in consultation with the local Health Units.

If your child is absent from school due to an illness that includes common COVID-19 symptoms, parents/guardians will be sent an attestation form from the school. Please complete the form prior to sending your child back to school. Return to school can only occur when you have followed the necessary local health unit protocols for your child to be back at school.
